"Cannot find the phone book entry."

This error is most usually a Windows NT 4.0 Error

First thing I'd try would be to create a new connectoid.

If building a new connectoid doesn't work, reboot the computer and try it again... If this still doesn't solve the problem, uninstalling and reinstalling Dial Up Networking (make sure you have your Windows NT 4.0 CD handy).
